For example, Health Insurance, Dental Insurance, Vision Insurance, Life Insurance, 401 (k), Flex Spending/Cafeteria Plan, FMLA acknowledgement & additional Read and Sign Forms. |

One-Time Setup Fee
With 321Forms, we do the heavy lifting for you! This initial fee will cover setting up your company's account, adding your company's unique onboarding forms to the system, and ensuring secure access to your company's employee data.
Monthly Subscription
In addition to the one-time setup fee, your monthly subscription will provide:
- Unlimited use of 321Forms for onboarding new hires, status changes and open enrollment
- Unlimited online storage and retrieval of employee data
- Ability to create new employee accounts and archive inactive employee accounts as necessary
- Two hours of training/technical support during setup and installation
- Technical support between 8am to 5pm Mountain Time
- High grade encryption and industry standard SSL (secure socket layer)
- Seamless upgrades to the system, including latest changes on federal and third party forms
- Unlimited use of the 321Forms Data Export feature
